To generate an annotated CRF, select the Form Builder option from either the Website Host or the Study menu as shown below in following screen captures.



When the application is opened, the Form Builder page is displayed. A form can be selected for annotation by clicking the Select control for the desired form from the Existing Forms data table as shown in the figure below.


Clicking the Select control loads the form into the Form Builder body/grid. The Links panel is located below the Existing Forms data table. The Annotate link (highlighted in the figure below) is used to generate an annotated PDF document for the currently selected form.


Clicking the Annotate link tells the system to generate a PDF version of the selected form. Once the system has completed generating the PDF version of the CRF, the system will prompt the user to save or open the downloaded Annotated PDF document. The Annotated PDF document shows the layout of the CRF with a field number displayed in red next to each field as shown in the figure below. The field number corresponds to the Field Number displayed on the Form Summary document that accompanies the CRF.

The Annotated PDF document header contains fields for the Study [name], [selected] Form, Version [applicable to the annotated form being displayed], Validated [Status of the form being displayed], Date [the Annotated PDF was produced], and Page information.


In addition to the form layout page(s) with the field number identifiers, the Annotated CRF also includes a Form Summary page(s) that provides the Form Name, Form Type, Number of Data Fields and the Total Fields contained on the form. Below the Form Summary is another table that provides additional details for each field as shown in the figure below. 


The table provides additional information about each field including the Field Number, Label associated with the field if applicable, Name of the field, Type of field, Length of the field, Choices for the field if applicable and any Conditional Actions.
